“The Chronic DiseasePrevention and Control Program (CDPCP) will be allocating funding to up to 8 primary care,internal medicine, family medicine or rural health clinics for work with hypertensive patients in the following areas:Team-Based CareElectronic Health Record IntegrationSocial Determinants of Health Screening and ReferralsSelf-Monitored Blood Pressure Program DevelopmentThis funding will total $25,500 per clinic for a 1-year performance period from 08/01/2024-07/29/2025. Clinics will be expected to submit several deliverables including a pre-award Scan and Plan, which is a tool for getting an accurate assessment of the clinics current practices, a complete action plan with SMART objectives, participate in check-in calls with the Heart Disease and Stroke team, quarterly progress reports, mid-year and end of year data, and participate in an evaluation session with the CDPCP Evaluator at the end of the grant period. This work is intended to address any gaps in care surrounding the above areas and improve the overall hypertension control rate of the clinic. All action steps must be evidence-based to ensure the best outcomes for Nebraskans.
